All requests for medical records are processed in compliance with the Privacy Act, DoD 6025.18-R and AFI 33-332.

MINIMUM NECESSARY RULE
DoD 6025.18-R C8.2.

C8.2.1. Standard: Minimum Necessary. When using or disclosing protected health information in any form or when requesting protected health information from another covered entity, a covered entity shall make reasonable efforts to limit the use, disclosure, or request of protected health information to the minimum necessary to accomplish the intended purpose of the use, disclosure, or request. The "reasonable efforts" standard applies to the implementation specifications in sections C8.3., C8.4.

Requesting Records

REQUESTING RECORDS IN PERSON

Requesting medical records in person is the easiest way to obtain specific information you need. Small requests can be processed on the spot.

Please come to the Patient Administration front desk in the main lobby of the hospital next to the barber shop.

Hours of operation are from 0700 to 1600 on weekdays.

Requesting a complete copy of your medical records may take up to 12 weeks

PLEASE NOTE:

Patients over 18 years of age are required to complete their own requests unless a power of attorney is presented.

REQUESTING RECORDS VIA FAX

Facsimile requests can be sent to 702-653-2103

To obtain a copy of medical records via fax, please send a signed authorization to the above fax number.
